Fall, tackle football athletes MUST complete the CFA registration form. Please click on the button above and print the CFA registration form. You will need to fill this out as this goes into the official CFA Roster Book. You will need to get a physician to sign the Physician's affidavit. This is the most important part of this document! 

 

ALL PAPERWORK MUST BE TURNED IN BEFORE ANY FOOTBALL PLAYER RECEIVES EQUIPMENT OR PARTICIPATES IN PRACTICES.

EQUIPMENT HANDOUT WILL BE THE WEEK OF JULY 21, SO PLEASE SUBMIT PAPERWORK ASAP.

 

That means this form must be completed and new players must turn in a photocopy of their birth certificate before they receive their equipment to practice. CFA physical forms, birth certificates, and any registration questions can be emailed to nlmidgetfootball@gmail.com.
