Registration Information
Registration for the 2026 season is now open! Early registration pricing is valid until May 1, 2026.
ONLY tackle football players need to complete the additional CFA form, and provide proof of 2026-27 school enrollment. Documents must be emailed to nlmidgetfootball@gmail.com, with the athlete's name in the subject of the email. Tackle football players who are new to the NLMFA must also email a copy of their birth certificate.
*These documents must be received prior to an athlete receiving equipment and participating in practices.*
Fall, tackle football athletes MUST complete the CFA registration form. Please click on the button above and complete the CFA registration form. You will need to get medical provider to sign the Physician's affidavit. This is the most important part of this document!
ALL PAPERWORK MUST BE TURNED IN BEFORE ANY FOOTBALL PLAYER RECEIVES EQUIPMENT OR PARTICIPATES IN PRACTICES.
EQUIPMENT HANDOUT WILL BE IN MID JULY, SO PLEASE SUBMIT PAPERWORK ASAP.
That means the CFA form and school enrollment verification must be completed and new players must email their birth certificate before they receive their equipment to practice. CFA physical forms, birth certificates, and any registration questions can be emailed to nlmidgetfootball@gmail.com.
This form does not pertain to cheerleaders or Itty Bitty football.
